Payment Agreement: I am submitting my deposit today and I agree to make monthly payments for Shakti Movement Medicine Training. First payment installment will be paid on or before 04/01/23, second payment on or before 05/01/23, third payment on or before 06/01/23, and fourth payment by 07/01/23. I understand that failure to submit my monthly payment installments on time will incur a late fee of 10%. Removal from the program will result if payment is past due for 30 days.
